I am a bit lost with this game. Got the spatula, ice cube and alarm clock but still the chest won't open...

Go upstairs and use the spatula to open the trunk.

In the level with the chiropractor, just walk outside of his office and walk to the left side of the yard. You will see a sea urchin (round black object) laying on the ground. You can get a spine from it without going through the maze.
