The Exorcist is great. You can find enough humor in it to offset the scariness. Well, at least for me, it's hard not to laugh at some the girl's antics while possessed, as gross as some of them are.
Although when I went to see the rereleased extended version in the movie theater I did have to apologize to my neighbour for grabbing on to him numerous times during the film. Luckily he was my friend and not a total stranger.
By the way, the original trailer for the Exorcist is one of the greatest trailers I have ever seen.
I was just thinking that "Marathon Man" is pretty damn scary, especially the scene where Dustin Hoffman is taking a nice, calm bath and hears whispers on the other side of the bathroom door. What happens next is just too scary and ingenious to get into. Also, I would think the scene with Hoffman and Laurence Olivier's dentist drill might cause some people to hide their eyes and cover their ears.

I am very surprised that no one has mentioned any of the Hannibal Lector movies. Red Dragon, Hannibal, and Silence of the Lambs will keep you up for days. I recently saw Gothica. It is pretty disturbing but not nearly as bad as the films based on Harris's books. I get chills just thinking about the movie. If you have not ssen all three of these you should try and rent them if you need a good scare or a few sleepless nights.
One of the scariest movies I have ever seen is Outbreak. This movie shows a whole lot of my fears and is truly frightening at points, even though it's not supposed to be a horror film. The U.S. military holding back vaccines to control chemical weapons is realistic and terrifying. Them prefering to obliterate the town to keep their weapon instead of offering to help and cure people was frighteningly plausible. The scene in the movie theatre with the guy coughing and then following the virus as it goes airborne through the crowd is a truly scary thing. These are imaginable to me, where a lot of "scary" things aren't.

I just saw "The Shining" at 1:30 yesterday (or would that make it today) morning. I have never in my life been so scared!! I just loved how Kubrick created tension by explaining things little by little. A newer talent who I think shares this gift is (and Im gonna get flamed like hell for this comment) M. Night Shyamalan. Watching the little child talk to Tony the boy who lives within his mouth reminded me a lot of all the wonderful creepy performances given by the children in Shyamalan's films.

I still get the willies when I drive by the hospital here in St. Louis (Alexian Brothers) where the (alleged) events of the Exorcist happened. The building was demolished, and rebuilt, but the site freaks me out. And I'm by no means a theist. But stories about how weird and creepy the abandoned floor of the hospital was still give me the creeps.
I was TERRIFIED of the Amityville Horror when I was a kid, but it doesn't freak me out as much now.

Ive seen a lot of scary movies including all those considered the best and almost all of them didnt scare me but the one that scared me the most by far was the Blair Witch Project. Its so realistic and freaky. Its also really creative.

Try Black Christmas or When a Stranger Calls for scary slasher films.
Try The Entity (or hunt down a tv movie called The Haunted-which features a ghost raping the father of a family and changing face while doing so-ultra freaky) or The Eye for a freaky ghost movie.
Try Cradle of Fear for a (kinda) freaky low-budgeter.
