I use Blockbuster Online. I pay $17.99 + tax and get three rentals at a time, always of movies I can't find at any stores. Then I return those to my local store and trade each one in for a free new release. About two days later, I get another "obscure" movie in the mail for each one I turned in to the store. It works out very well. I get about 15-20 movies a month, averaging about $1 per movie, and I can keep the mailed ones as long as I want and the ones from the store for 9-15 days.
The actual rental fees around here, in Orange County, CA, vary. If I rented at the store w/o the Online program, the cost would be $4 a pop, although I used to get a free, non-new release with each paid DVD. The local grocery store has just changed their policy to $1 per night for ALL movies.
